It is a pleasure for me to give this opening address to the Royal Institute of Philosophy Conference on ‘Explanation’ for two reasons. The first is that it is succeeded by exciting symposia and other papers concerned with various special aspects of the topic of explanation. The second is that the conference is being held in my old alma mater, the University of Glasgow, where I did my first degree. Especially due to C. A. Campbell and George Brown there was (...) in the Logic Department a big emphasis on absolute idealism, especially F. H. Bradley. My inclinations were to oppose this line of thought and to espouse the empiricism and realism of Russell, Broad and the like. Empiricism was represented in the department by D. R. Cousin, a modest man who published relatively little, but who was of quite extraordinary philosophical acumen and lucidity, and by Miss M. J. Levett, whose translation of Plato's Theaetetus formed an important part of the philosophy syllabus. (shrink)

Imagine that we are considering whether our healthcare system (or insurer) should fund treatment or procedure X. One factor that may be cited is that of so-called ‘medical necessity’. The claim would be that treatment X should be eligible for funding if it is medically necessary, but ineligible if this does not apply. Similarly, (and relevant to the debates in this special issue), if considering whether a particular treatment should be ethically and/or legally permitted, we may wish to distinguish between (...) cases where the treatment is medically necessary, and those were it is not. But what do we mean by this concept? Here I will propose and briefly defend one plausible and practical definition. (shrink)

Basic disagreements about what makes human life valuable hinder use of the concept of futility to decide whether it is appropriate to continue life support for one in a permanent state of unconsciousness, or to provide intensive medical care to one in the last stages of a terminal illness (the “paradigm cases”). Triage planning (the process of establishing criteria for health care prioritization) is an attractive alternative framework for addressing the paradigm cases. Triage planning permits society to see the cases (...) in the context of diverse moral perspectives, limited resources, and competing health care demands. Furthermore, at least one essential question posed by the paradigm cases is whether treatment is wasteful, and triage planning is a useful model for identifying and eliminating wasteful medical care. The authors describe how triage planning can be implemented to address the paradigm cases, and conclude that it offers one way of moving debate about these cases beyond futility. (shrink)

BackgroundClinical Ethics Support (CES) services are offered to support healthcare professionals in dealing with ethically difficult situations. Evaluation of CES is important to understand if it is indeed a supportive service in order to inform and improve future implementation of CES. Yet, methods to measure outcomes of CES are scarce. In 2014, the European Moral Case Deliberation Outcomes Instrument (Euro-MCD) was developed to measure outcomes of Moral Case Deliberation (MCD). To further validate the instrument, we tested it in field studies (...) and revised it. This paper presents the Euro-MCD 2.0 and describes the revision process.MethodsThe revision process comprised an iterative dialogue among the authors as Euro-MCD-project team, including empirical findings from six Euro-MCD field-studies and input from European experts in CES and theory. Empirical findings contained perceptions and experiences of MCD outcomes among healthcare professionals who participated in MCDs in various settings in Norway, Sweden and the Netherlands. Theoretical viewpoints on CES, literature on goals of CES and MCD and ethics theory guided the interpretation of the empirical findings and final selection of MCD outcomes.ResultsThe Euro-MCD 2.0 Instrument includes three domains: Moral Competence, Moral Teamwork and Moral Action. Moral Competence consists of items about moral sensitivity, analytical skills and virtuous attitude. Moral Teamwork includes open dialogue and supportive relationships and Moral Action refers to moral decision-making and responsible care. During the revision process, we made decisions about adding and reformulating items as well as decreasing the number from 26 to 15 items. We also altered the sentence structure of items to assess the current status of outcomes (e.g. ‘now’) instead of an assumed improvement over time (e.g. ‘better’) and we omitted the question about perceived importance.ConclusionsThe Euro-MCD 2.0 is shorter, less complex and more strongly substantiated by an integration of empirical findings, theoretical reflections and dialogues with participants and experts. Use of the Euro-MCD 2.0 will facilitate evaluation of MCD and can thereby monitor and foster implementation and quality of MCD. The Euro-MCD 2.0 will strengthen future research on evaluation of outcomes of MCD. (shrink)

In this article, I present a critical literature study of the theoretical approach of practical theologians in South Africa to our discipline, in honour of Yolanda Dreyer on her 60th birthday. Some of my colleagues' approaches at the universities of Stellenbosch, Free State, Pretoria, Unisa and NWU are discussed. All of them work with practical theological hermeneutics. The basic hermeneutic approach of Daniël Louw is widened with an integrated approach by Richard R. Osmer in which practical theology as a hermeneutic (...) discipline also includes the empirical aspect which the action theory approach has contributed to the discussion. After discussing Louw's basic hermeneutic approach, all the other colleagues who basically accepted Osmer's approach in their publications are discussed. Important and new ground is being broken by contemporary colleagues, including research in public practical theology, without neglecting the focus on Christian congregations, and new work is being done at the grass roots level of African issues in our country. As an immediate reply, meanwhile, I should like to make a remark or two about what is perhaps the central issue raised in the foregoing Comment. This is whether Price's dispositional reduction, or anyone's dispositional reduction can adequately render the act of thinking. Alternatively, the question could, I think, be stated in more presently-favoured language. One could ask whether statements about universals, thoughts, concepts, or ideas can be logically deduced from dispositional statements not containing these "mentalistic" terms. If one is (...) inclined to prefer this more lengthy formulation one may then take this to be my meaning; whenever "dispositional solution" or "dispositional reduction" is mentioned one may translate it into this longer form.
